Sunset Crossing Travel Clinic provides comprehensive educational and
immunization services to travelers. Individuals seen in the
clinic will receive the following services.

=Individual assessment of immunization and health care needs based on
travel plans and personal health history.

=Information regarding commonly encountered health problems and methods
of prevention and treatment.

=Prescriptions of appropriate medications needed for prevention or treatment
of travel-related illnesses.

=Any necessary laboratory services for any titers or required blood tests.

Clients are charged a one time (per trip) consultation fee
in addition to the cost of any immunizations.

Clients are responsible for all fees on the date of service.
We accept Visa and Mastercard.

=Records of previous immunizations.
=Detailed itinerary of your trip and dates of stays in each destination.
=A list of any medications you are currently taking.
=Some immunizations must be taken in a series and will necessitate return visits.
